I agree to abide by the rules and regulations as set forth by the Port Wentworth Leisure Services Department.

I understand that the Port Wentworth Leisure Services Department staff will assign my child to a team. I fully accept the decision of the Leisure Services Department staff regarding team selections.

I will conduct myself with a positive attitude towards Leisure Services Department Staff, coaches, opposing team, fans and officials during the course of the season. I understand that failure to follow this principle may lead to punishment by the Port Wentworth Leisure Services Department staff, which may include but not limited to suspension from the Port Wentworth Leisure Services Department events. I support the Port Wentworth Leisure Services Department youth sports philosophy, which is based on participation, fun, physical fitness, and health, skill development, teamwork, fair play, family involvement and volunteer leadership.

I hereby certify that my child is in normal health and capable of safe participation in the youth sports program. I understand that injuries may occur and that Port Wentworth Leisure Services Department Does Not carry supplement insurance. I also understand that the Port Wentworth Leisure Services Department may require some or all of the sports equipment assigned to my child to be turned in at the conclusion of the season. Failure to do so will result in a fee being charged to cover the cost of the equipment, all printing, charges, set-up and all shipping incurred by The City of Port Wentworth Leisure Services Department.

Privacy Statement:

The City of Port Wentworth will not collect any personal information about you when you visit our website unless you choose to provide that information to us voluntarily. We do not give, share, sell, or transfer any personal information to a third party, unless we are legally required to do so.

Refund Policy:

The City’s Leisure Services Department does not refund to Credit Card.  Any refunds for Class, Program, Rental or Purchase that has not yet begun or used will be refunded in full through the City’s Accounts Payable Department by Check, refundable to the payer.  Any refunds for Classes, Programs or Products that have begun or have in any way been used, will be considered on a case by case basis.

Assumption of Risk and Waiver of Liability Relating to

Coronavirus/COVID-19

            The novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization.  COVID-19 is extremely contagious and is believed to spread mainly from person-to-person contact.  As a result, federal, state, and local governments and federal and state health agencies recommend physical distancing and have, in many locations, prohibited the congregation of groups and people.

            The City of Port Wentworth (City) has put in place preventative measures to reduce the spread of COVID-19; however, the City cannot guarantee that you, your children, or any other person, will not become infected with COVID-19.  Further, attending City sponsored activities could increase your risk and your child or children’s risk of contracting COVID-19. 

            By signing this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that my child or children and I may be exposed to or infected by COVID-19 by attending City activities and that such exposure or infection may result in personal injury, illness, permanent disability, and death.  I understand that the risk of becoming exposed to or infected by COVID-19 at City activities may result from the actions, omissions, or negligence of myself and others, including but not limited to City employees, volunteers, and program participants and their families. 

            I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to my child or children or myself, including, but not limited to, personal injury, disability, death, illness, damage, loss, claim, liability, or expense or any kind, that I or my child or children may experience or incur in connection with my child or children’s attendance at City activities or programming.  On my behalf and on behalf of my child or children, I hereby release, covenant not to sue, discharge, and hold harmless and indemnify the City, its employees, agents, and representatives, of and from any and all claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto.  I understand and agree that this release includes any claims based on the actions, omissions, or negligence of the City, its employees, agents and representatives, whether a COVID-19 infection occurs before, during, or after participation in any City program. 

I further acknowledge and agree that if my child exhibits any signs or symptoms of COVID-19  or any other illness  during the time when my child is left in the City’s custody, my child will be immediately segregated until he/she is picked up by the parent or responsible party.  I also agree that if notified of such a concern by a City employee, that I will immediately pick up my child to protect my child and other persons which whom he/she may be in contact.
